Walnut Street Community Health Center provides a variety of medical, dental, mental health, and supportive servcies for patients from infants to seniors.

Walnut Street Family Practice:

Walnut Street Family Practice is accepting new patients to include Medicare, Medicaid, most private insurances, and those without insurance. Services are offered for prevention and sick care to all members of the family. Our doctors are on call after hours. Hospitalist doctors care for our patients while in the Washington County Hospital.

Walnut Street Dental Practice:

Walnut Street Dental Practice accepts child and adults from the Walnut Street Family Practice. The Dental Practice cares for pediatric patients with Medicaid, Medicaid MCO, uninsured, and self pay. Dental services are made available to adults without dental insurance. The Dental Practice provides preventive care, restorative care, dentures, and extractions.

Mental Health Services:

The Walnut Street Family Practice has now integrated Mental Health Services. Currently, a therapist is available on a full time basis for therapy appointments for patients of the Family Practice. There are also appointments available for medication management with a professional psyciatric provider one half day a week.

Financial Assistance:

A sliding fee scale is based on household income is available to patients for medical, dental, and mental health services.

Supportive Services:

Patients may receive health education, as well as assistance with referrals, transportation, food, housing, and other non-medical needs. A representative from the Department of Social Services is located at the Center to assist with patient financial issues.
